COPFTO Step 5 Supervisor Summary Questionnaire

Step 5 Supervisor Summary Reports are designed to be completed after the probationer has been released from field training, at specific intervals, throughout the balance of the probationary period.

How this data collection form works:
– This form walks you through each section of the Step 5 Summary.
– You select Yes or No on including each section.
– If you select YES, additional questions may open up.
– If you select NO, we will hide that specific section.
– You can customize the title & description of each section.
– You can add custom sections to collect data your agency needs.
– Once Submitted, we customize the Step 5 Summary to meet your Agency’s needs.
