COPFTO Weekly Summary Questionnaire

Weekly Summaries are completed at the end of each work week.

How this data collection form works:
– This form walks you through each section of the Weekly Summary.
– You select Yes or No on including each section.
– If you select YES, additional questions may open up.
– If you select NO, we will hide that specific section.
– You can customize the title & description of each section.
– You can add custom sections to collect data your agency needs.
– Once Submitted, we customize the Weekly Summary to meet your Agency’s needs.

This section lists any big 12 tools used with a comment box for each,  and displays the activities you want to track.

This section provides text fields to document activity.

Custom Sections (COPFTO Enhancement)

Capture custom content your agency’s wants included in your Weekly Summary.

Files Section

This section allows you to upload and attach files to your Weekly Summary
For each custom section, please provide: Title (name of the section) & Instructions (a sentence directing the person on the content you want entered)

Lowest Create Role

This section determines the lowest role allowed to create a weekly summary. You selection depends on whom within your agency normally creates your weekly summaries.
